Personalized Care

Personalized care focuses upon treating people rather than diseases. Your treatment plan is individualized to address the specifics of your situation considered as a whole -- your health history, your genetic individuality, your bioenergetic constitution, your cultural identity, your social and financial circumstances, your beliefs about health and healing, and your treatment goals.

Advanced Access

Advanced Access refers an approach to scheduling that addresses your needs as promptly as possible. With Advanced Access, you are offered an appointment on the same day that you call. Advanced Access does not prevent you from scheduling appointments in the future. It simply guarantees a same day appointment if you want one.

Telemedicine

HeartSong uses a variety of electronic technologies to facilitate communication unless a situation is life-threatening.

If you believe that your life, or the life of someone else, is in danger, you should go directly to the nearest emergency room or dial 911.

Your personal physician will also give you a private email address for medical questions directly related to your treatment plan

2. Telephone: HeartSong's phone number is easy to remember -- 802-387-2345.

3. Website:  If you are currently receiving health care at HeartSong, you will have a personal password and login which will give you access to your dashboard where you will find the following:  (a) personal current and past treatment plans, (b) selected lab and test results, (c) self-management tools, (d) an interactive secure communication system to communicate with your physician, request referrals, renew prescriptions, and order nutraceuticals.  Internal pages contain various links to HeartSong email addresses for the general public.

4. Pager: For people who receive medical care at HeartSong, our in-house physician's pager number is on your personal dashboard.

5. Patient Fusion: Our web-based electronic patient portal allows  to access medications, diagnoses, and appointment times.

6. Fax: HeartSong's fax number is 802-387-5311
